If you wish to customize the report, first make your changes in Word and save or print it to a PDF. If you want to use a QR code, see the instructions below. Otherwise, you can remove the QR code and put your email in instead, asking people to contact you directly for a copy.

To create a QR code, first create a PDF, then go to a QR code site like QRCodeChimp or Canva.com to create a free QR code from your PDF.
